Social Prescribing

Social prescribing offers members practical support for managing their own mental health problems, encouraging individuals to take control through self-management. It enables the service to refer members to local and non-clinical services, supporting their mental health and wellbeing in a holistic way.

  • Offering information and advice about mental health, services, support, and self-management
  • Available by phone, text, online and face to face
  • Connects people with support and community resources
  • Motivates and encourages people
  • Directs people to specialist advice and support services, e.g. Citizens Advice, housing support

We offer up to 3 sessions as part of our social prescribing service. We will identify your needs and the services that can support you – we will refer you to this service. Once the referral has been made, we can support you in accessing the service, through providing additional reminders, checking in with you, encouragement or accompanying you to your first session – this is known as a warm handover.

Benefits of a warm handover: 

  • Helpful for people who may feel overwhelmed, isolated, or unsure about engaging with new services
  • Increases the likelihood of engagement with the service
  • Creates a smoother, more compassionate transition between services
  • Helps you to feel supported

How can I access it?

The HWN social prescribing service is a free service available to all residents in Haringey. We accept self-referrals and referrals from agencies. Please complete the referral form below. We will then contact you to complete an assessment.

Criteria to access this service are:

  • A resident of Haringey or registered with a Haringey GP
  • You must be aged 18 and over
